For safety and security reasons, only clients receiving services will be allowed to go into the clinic area.

Children will be seen one at a time. Therefore, clients must bring someone of an appropriate age and maturity to help so their children will be supervised at all times.

Care Coordination for Children (CC4C)

Care Coordination for Children (CC4C) is a program that strives to improve healthcare access for families with children. 

Our CC4C Care Manager will help families find medical care and other community services. Children from birth to 5 years of age that meet the following criteria will benefit from services:

  • Are a high cost/high user of Medicaid services
  • Have special health care needs
  • In foster care with no medical home
  • Are exposed to toxic stress in early childhood
