Child Service Coordination

Child Service Coordination focuses on infants and children up to age 5 who have been identified with developmental and/or chronic health problems or who are at risk for such problems.  A social worker ensures that the child receives appropriate health and education services as well as referrals to other supported resources.

For safety and security reasons, only clients receiving services will be allowed to go into the clinic area.  Children will be seen one at a time.  Therefore, clients must bring someone of an appropriate age and maturity to help so their children will be supervised at all times.
