Site Evaluation for Septic Tank System

• (New Lots)/IP/Denial - $60.00

• Re-Visit fee for site not ready (new or existing lot evaluations) - $25.00

• Construction Authorization/OP new installation - $60.00

• Permit Revision Fee (if site or design flow changes) - $75.00

• Septic Tank System Inspections Repair Permit (requested by owner) - $50.00

• Repair Permit (by complaint investigation - violation) - $75.00

• Relocation/Existing System Inspection - $50.00

• Mobile home park authorization/connection - $20.00

 

Operation Permits for Systems Under Management Entity

• Less that 480 gallons per day - $60.00

• Greater than 480 gallons per day - $120.00

 

Water Sample Fees

• Bacteriological/Required Well Inspection - $40.00

• Chemical - $30.00

• Pesticide - $50.00

• Petroleum - $50.00

• Lead - $10.00

• Nitrates - $25.00

 

Private Drinking Well Fees

• New Private Drinking Water Well Permit - $200.00
  (Includes sampling and inspections required)

 

Swimming Pool Permits

• Annual Fee (Public Pool) - $100.00

• Plan Review (Public Pool) - $150.00

• Any Additional Pools - $100.00

 

Tattoo Permits

• Annual Artist Fee - $200.00

 

Food and Lodging

• Food Safety Manager's Course - $100.00

For safety and security reasons, only clients receiving services will be allowed to go into the clinic area.  Children will be seen one at a time.  Therefore, clients must bring someone of an appropriate age and maturity to help so their children will be supervised at all times.
