Postpartum/Newborn Home Visits

Home visits by a registered nurse are made to mothers and babies who are on Medicaid and/or received prenatal or maternity care coordination services from the Health Department. The nurse can provide confidential services in the home setting and answer questions that often arise after hospital discharge. Services are available upon request of the client or medical provider.

For safety and security reasons, only clients receiving services will be allowed to go into the clinic area.  Children will be seen one at a time.  Therefore, clients must bring someone of an appropriate age and maturity to help so their children will be supervised at all times.
